How to complete a Home Oxygen Order Form

Completing a Home Oxygen Order Form (HOOF) is an essential process to order home oxygen

Several important requirements must be fulfilled to prevent delays and ensure that the HOOF is not rejected by the contractor.

It is recommended that a Home Oxygen Consent Form (HOCF) is completed before a HOOF. The HOCF allows you to share relevant information with the local home oxygen provider. Please note that faxed Home Oxygen Order Forms are no longer accepted. You need to submit the form through the BOC Home Oxygen Portal. For detailed information on how to submit a HOOF, please see the Home Oxygen Order Help Guides below.

When completing the HOOF, ensure that the form is as detailed as possible and fill in all mandatory parts, marked with an asterisk (*). Patient and carer details, clinical details, and patient's GP information are compulsory.

Section 7 to 9 has mandatory fields, such as flow rate, equipment type, and delivery mode, among others. Ensure that the flow rate is given in litres per minute with a digit to the left of the decimal point. If you require a mask, specify the mask type and % delivery (e.g. 28%).

The appropriate delivery mode can either be nasal cannula or mask. Select the urgent supply option only if the situation demands it. Mark one box only in section 10, selecting the most appropriate.

If you have a specific request, add it to section 11. You can use section 12 if you need queries to be specifically directed somewhere.
